In creating Hope Hospice, Partners Hope Foundation recognized Missoula’s rich history of providing end-of-life services and sought to partner with existing healthcare and service providers to build upon what has been accomplished and to meet the expanding needs of our Montana community. Today, through a deep commitment to collaboration, Hope Hospice is a center for hospice and end-of-life care and a community hub for support and educational resources around end of life.

Partners In Home Care is the contracted clinical care provider of Hope Hospice. Their team, including medical director, nurse practitioners, pharmacist, nurses, social workers, chaplains CNAs and administration, provide day-to-day patient care as well as family support and bereavement services at the center.
Established in 1987, Partners In Home Care is locally owned and the largest nonprofit home health and hospice care provider in Western Montana. PIHC’s history of commitment to the patients and families they serve is the foundation of their steadfast reputation in our community. Through our partnership, built on providing compassionate care for our Montana community, Partners Hope Foundation operates the center with PIHC to provide a place where the end-of-life experience can be enriching and transformative.
All Nations Health Center, established in 1970, provides a comprehensive suite of culturally attuned services and programs for elders, children, adults and families.
Partners Hope Foundation is committed to working with and learning from All Nations to offer Native people and Montana patients and families culturally based, holistic care. Collaborations to date have included installing smudge fans in sanctuary spaces and each patient room at Hope Hospice, including native plants and grasses in landscaping, and infusing the artwork of many local indigenous artists into the fabric of the center.
Missoula Aging Services has been promoting the independence, dignity and health of Montanans since 1982, empowering them to age at home and engage in community life.
MAS provides a wide range of programs and services, including expert assistance with Medicare/Medicaid and other benefits, Meals on Wheels, support for family caregivers, care management for people with dementia or other complex conditions, money management, volunteer opportunities, the Veteran Directed Care program, and more.
Partners Hope Foundation is actively pursuing partnerships throughout the region to create seamless access to end-of-life services and provide increased support and educational opportunities for our Montana community.
