Hope Hospice Center provides Montana patients and their loved ones 24-hour end-of-life care in a warm, comforting and state-of-the-art environment. Partners In Home Care’s compassionate team of hospice professionals focuses on intensive symptom management, supporting patients with complex needs that cannot be safely or effectively met at home or in a facility.

Patients are evaluated on a case-by-case basis and must meet these criteria for hospice care:
The patient is experiencing acute, uncontrolled symptoms requiring intensive medical management that cannot be delivered in a home or facility setting, such as uncontrolled pain, severe dyspnea (shortness of breath), terminal agitation or active delirium, intractable nausea, or vomiting or complex wound care requiring continuous skilled support.
The patient cannot remain at home but does not require intensive symptom management.
The patient is currently receiving hospice care through a licensed hospice agency and the patient’s primary caregiver needs respite. The patient can be transported to Hope Hospice for short-term admission of up to five consecutive days, allowing the caregiver the opportunity to attend to their own well-being while ensuring their loved one continues to receive high-quality care.

Financial assistance is available as needed through Partners Hope Foundation’s Compassionate Care Fund, which helps cover room and board fees on a sliding scale.
